Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc.
must be in a manner that is courteous, respectful, cooperative and professional.

PRIMARILY SERVES:
Neonatal (0-28 days) Infant (28 days-2 yr.
) Pediatric (2-12 yrs.
) Adolescent (12-20 yrs) Adult (21 64 yrs) Geriatric (65+) Families Staff Community Agencies Physicians Visitors Volunteers EQUIPMENT/MACHINERY USED Office Equipment PREFERRED QUALIFICATIONS 1.
One year clerical experience in a medical environment required.

Experience in hospice/home health admissions preferred.
DUTIES AND RESPONSIBILITIES Performs intake and processes referrals.
Screens referrals for eligibility for hospice services.
Arranges DME, supplies, etc.
Liaisons with MDs, referral sources, etc.
If LVN, arranges medications.
Inputs data, POC, demographics, etc.
into Suncoast.
Schedules admission visits.
Provides staff/IDT with patient admission information and documentation.
Performs data entry (excel, EMR) and maintains records and information for patients being admitted to Hospice Austin.
Processes medical records requests MINIMUM TRAINING, EDUCATION AND EXPERIENCE REQUIREMENTS High School diploma or equivalent.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
